Thank you for signing up for a COVID-19 Family Support Kit. You can pick up your kit at 133 E. Davis Street in downtown Burlington (across from May Memorial Library) on:

  • Friday, November 13 between 12:30 and 6 p.m.

  • Wednesday, November 18th between 8 and 10 a.m.

  • Friday, November 21 between 9 a.m. and 11 a.m.

If you want your kit to be delivered to you, please complete the following form. You do not need to fill out the form if you plan to pick up your kit.
